This module builds on the knowledge and skills developed in stage one and two. You will have the opportunity to expand your range of assessment skills and apply them under minimum supervision whilst managing and coordinating the care of people with learning disabilities with a range of conditions varying in complexity and in a range of settings. The module also assists you to develop skills in advanced care management and complex discharge or transfer planning.
